Are You Using the Re-Issued Department of Labor FMLA Forms?

Use of the expired FMLA forms may present a problem for an employer if a health care provider closely reads the fine print and wishes to find an excuse not to comply with the request. For this reason, it is recommended that employers use the updated forms to avoid this potential problem.

See our Issue of the Month, New Forms - No Change, posted on the Krukowski & Costello web page for more information concerning these re-issued forms and for updated form copies.
